If your EIP Card got destroyed, lost or stolen

If you received a destroyed card/mail, accidentally threw away the card or the card was stolen you can call and report your stolen/lost card. The card will be deactivated to prevent anyone from using it and you can request a replacement Card that will be sent to you at no cost.

Step 1: Call number #1-800-240-8100

Step 2: It will ask you which language English or Spanish

Step 3: It will ask you: Activate Card or Customer Service, press one; It will ask you if the card is damaged or stolen/lost, press two.

Step 4: It will ask the last six digits of your social security number, you will need to provide that information.

Step 5: It will ask your zip code.

There are a few more steps that we are unable to verify, however proceed through all the steps.

Once you have received a new card check out our vlog on activating your card. Please do not forget your PIN number. It is very IMPORTANT that you remember the PIN number.
